There are two kinds of problems, accidents and bullshit. It is imperative to understand first the difference between whether or not someone could have known to choose or avoid a particular action, then whether they had a responsibility to do so. If they were negligent either in action or inaction, that's the bullshit variety where correction is required. It's a rather precise formula.
